Azelastine

Nasal Conditions

Azelastine nasal spray is used to treat symptoms of seasonal allergic rhinitis, commonly known as hay fever, and perennial allergic rhinitis. It helps relieve the following symptoms:

• Sneezing
• Runny or itchy nose
• Nasal congestion
• Postnasal drip

It is also used to treat vasomotor rhinitis. This is a condition that causes a runny or stuffy nose but is not triggered by an allergy or an infection.

Eye Conditions

In the form of eye drops, azelastine is used to treat itching of the eyes associated with allergic conjunctivitis. This condition occurs when the eyes react to allergens such as pollen, pet dander, or mold.

Combination Use

Azelastine may be used alone or in combination with other medications. For example, it is sometimes available in a single nasal spray combined with a corticosteroid to provide relief for seasonal allergy symptoms that require multiple types of treatment.

Mechanism of Action

Azelastine belongs to a class of medications known as antihistamines. It works by competing with histamine for H1-receptor sites in the body. Histamine is a chemical released by the immune system when it encounters an allergen, such as pollen, mold, or pet dander. When histamine attaches to receptors in the nose or eyes, it triggers the physical symptoms of an allergic reaction.

Targeted Relief

Serious Side Effects

Although rare, some side effects can be serious. You should contact a healthcare professional immediately if you experience:

• Severe drowsiness or extreme fatigue
• Fainting or lightheadedness
• Fast or pounding heartbeat
• Shortness of breath or difficulty breathing
• Swelling of the face, lips, tongue, or throat
• Severe skin rash, hives, or itching
• Severe sores, ulcers, or crusting inside the nose

Azelastine may cause significant drowsiness. This effect can be worsened if the medication is taken with alcohol or other substances that cause sedation.

Drowsiness and Alertness

Azelastine can cause somnolence (drowsiness). Caution is advised when driving, operating heavy machinery, or engaging in hazardous activities until the effects of the medication are known.

Alcohol and Other Medications

Combining azelastine with alcohol or other central nervous system depressants may further impair alertness and physical coordination. It is generally recommended to avoid alcohol while using this medication.

Local Irritation

• Nasal Spray: Some users may experience nasal sores, nosebleeds, or ulcerations. A healthcare provider should monitor for these issues during long-term use.
• Eye Drops: Users should avoid wearing contact lenses if their eyes are red. If using the drops while wearing contacts, wait at least 10 minutes after application before reinserting lenses.

Pregnancy and Breastfeeding

The safety of azelastine during pregnancy or while breastfeeding has not been fully established. It should be used only if the potential benefit justifies the potential risk to the fetus or infant.

Allergic Reactions

Azelastine is contraindicated in individuals with a known hypersensitivity to azelastine hydrochloride or any of the inactive ingredients in the formulation.

Pregnancy

There are no adequate and well-controlled studies on the use of azelastine in pregnant women. In animal studies, very high doses of the medication—significantly higher than the standard human dose—were associated with developmental issues, including skeletal abnormalities and lower birth weight. Because animal research does not always predict how a drug will affect humans, azelastine should be used during pregnancy only if a healthcare provider determines that the potential benefit justifies the potential risk to the fetus.

Breastfeeding

It is currently unknown whether azelastine is excreted in human breast milk. Because many medications can pass into milk and potentially affect a nursing infant, caution is advised. Individuals who are breastfeeding or planning to breastfeed should consult with their healthcare provider to discuss the risks and benefits of using azelastine.

Nasal Spray

Azelastine is approved by the U.S. Food and Drug Administration (FDA) for the management of symptoms related to allergies. Its approved uses vary by the method of administration:

• Nasal Spray: This form is approved to treat seasonal allergic rhinitis in adults and children, perennial allergic rhinitis, and vasomotor rhinitis (non-allergic congestion and runny nose).
• Ophthalmic Solution: These eye drops are approved to treat itchy eyes caused by allergic conjunctivitis.

Regulatory Status

Azelastine is available in several forms, and its regulatory status depends on the formulation:

• Prescription Status: Higher concentrations and certain combination products that include other medications, such as steroids, are available by prescription only.
• Over-the-Counter (OTC) Status: Some nasal spray versions of azelastine have been approved for over-the-counter sale, allowing consumers to purchase them without a prescription for the relief of allergy symptoms.

Safety and Warnings

The FDA has not issued any boxed warnings for azelastine, which are the most serious type of warning for medications. There are also no Risk Evaluation and Mitigation Strategies (REMS) required for this drug. Regulatory oversight continues through standard post-marketing surveillance to monitor for any new safety concerns.

How Azelastine Nasal Spray Works
