Prevention of Blood Clots

Boxed Warning: Spinal or Epidural Hematomas
When Lovenox is used in patients undergoing spinal anesthesia or spinal puncture, there is a risk of developing a blood clot (hematoma) around the spinal cord. These clots can cause long-term or permanent paralysis. The risk is higher for patients with indwelling epidural catheters, those taking other medications that affect blood clotting (such as NSAIDs or platelet inhibitors), or those with a history of spinal surgery or deformity.
Risk of Bleeding
As an anticoagulant, Lovenox increases the risk of serious and sometimes fatal bleeding. Healthcare providers typically monitor for signs of internal bleeding, such as unusual bruising, blood in the urine or stool, or coughing up blood. Certain factors increase this risk, including:
- Severe kidney impairment
- Low body weight
- Recent surgery or trauma

Heparin-Induced Thrombocytopenia (HIT)
Lovenox can cause a serious condition called heparin-induced thrombocytopenia, which is a significant drop in platelet counts caused by an immune system reaction. This condition can lead to the development of new blood clots. Platelet counts are usually monitored regularly during treatment, and the medication must be stopped if HIT is suspected.
Mechanical Heart Valves
The use of Lovenox for preventing blood clots in patients with mechanical prosthetic heart valves has not been adequately studied. There have been reports of valve thrombosis in pregnant women with mechanical heart valves who were treated with enoxaparin, sometimes resulting in death of the mother or fetus.
Interchangeability
Lovenox is a low molecular weight heparin and is not interchangeable unit-for-unit with heparin or other low molecular weight heparins. Each of these medications has different chemical properties and dosing requirements.
Contraindications
Lovenox should not be used in patients with active major bleeding, a history of heparin-induced thrombocytopenia, or a known hypersensitivity to enoxaparin, heparin, or pork products.
Administration
Lovenox is usually administered as an injection under the skin (subcutaneously). It is most commonly injected into the fatty tissue of the stomach area. The injection site should be rotated with every dose, choosing a different spot on either the left or right side of the abdomen, at least two inches away from the belly button.
Injection Steps
- Wash your hands and clean the skin at the injection site with an alcohol wipe.
- If using a pre-filled syringe, do not press the plunger to remove the air bubble before injecting unless specifically instructed to do so by a healthcare professional.
- Pinch a fold of skin between the thumb and forefinger. Insert the needle straight into the skin fold at a 90-degree angle.
- Push the plunger down as far as it will go to deliver the full dose.
- Remove the needle and release the skin fold. Do not rub the injection site after the needle is removed, as this can lead to bruising or bleeding.
Missed Dose
If a dose of Lovenox is missed, it should be taken as soon as it is remembered. If it is nearly time for the next scheduled dose, the missed dose should be skipped and the regular schedule resumed. Two doses should not be taken at once to make up for a missed one.
Safe Disposal
Used needles and syringes must be placed in a puncture-resistant sharps container immediately after use. These items should not be disposed of in the regular household trash or recycling bins.

Pregnancy
