Indications for Use

Ruconest is used to treat acute attacks of hereditary angioedema (HAE). HAE is a rare genetic condition that causes episodes of severe swelling in various parts of the body, such as the hands, feet, face, digestive tract, or airway.

This medication is specifically indicated for:

• Adult patients experiencing an acute HAE attack.
• Adolescent patients experiencing an acute HAE attack.

Ruconest works by replacing a protein called C1 esterase inhibitor. In people with HAE, this protein is either missing or does not function correctly, leading to the uncontrolled swelling associated with the condition.

Important Considerations

Ruconest is intended for the treatment of active symptoms once an attack has begun. It is not used for the routine prevention of HAE attacks. Additionally, the effectiveness of Ruconest has not been established for treating laryngeal attacks, which involve swelling of the throat or airway.

Restoring Protein Levels

People with hereditary angioedema (HAE) have a genetic deficiency or a malfunction of a protein called C1 esterase inhibitor. This protein plays a critical role in regulating the immune system and inflammatory responses.

Controlling Swelling

When C1 esterase inhibitor levels are too low, the body produces too much of a chemical called bradykinin. Excessive bradykinin causes blood vessels to widen and leak fluid into nearby tissues, leading to the rapid and severe swelling seen during HAE attacks.

Ruconest works by:

• Replacing the missing protein: It provides a recombinant (man-made) version of the C1 esterase inhibitor that the body is lacking.
• Regulating inflammation: Once in the bloodstream, the medication helps control the pathways that lead to inflammation.
• Stopping bradykinin production: By restoring protein levels, it prevents the further release of bradykinin, which stops the swelling from progressing and allows the body to begin recovering.

Serious Side Effects

Some side effects can be severe and require immediate medical attention. These include allergic reactions and blood clots.

Allergic Reactions

Ruconest is derived from the milk of genetically modified rabbits. People with a known rabbit allergy should not use this medication due to the risk of severe allergic reactions (anaphylaxis). Symptoms of a reaction may include:

• Hives or skin rash
• Swelling of the face, lips, tongue, or throat
• Wheezing or difficulty breathing
• Chest tightness
• Dizziness or fainting

Blood Clots

Serious blood clotting events have been reported with the use of C1 esterase inhibitors. Seek emergency care if you experience:

• Pain, swelling, or redness in a leg or arm
• Sudden shortness of breath
• Chest pain
• Sudden weakness or numbness, particularly on one side of the body

Allergic Reactions and Anaphylaxis

Severe hypersensitivity reactions, including anaphylaxis, may occur during or after the administration of Ruconest. These reactions can be life-threatening. Because Ruconest is produced in the milk of transgenic rabbits, it must not be used by anyone with a known or suspected allergy to rabbits or rabbit dander. Symptoms of a serious reaction can include hives, rash, chest tightness, wheezing, or low blood pressure.

Blood Clotting (Thromboembolic Events)

Serious blood clots have been reported with the use of C1 esterase inhibitor products. The risk may be increased in individuals with certain pre-existing conditions or risk factors, including:

• A history of blood clots or cardiovascular disease.
• The use of indwelling central venous catheters.
• Prolonged periods of immobility.
• The use of high doses of the medication.

Monitoring for signs of thrombosis, such as swelling and pain in the limbs, shortness of breath, or sudden weakness, is recommended.

Limitations of Use

Ruconest is indicated only for the treatment of acute attacks of hereditary angioedema (HAE) in adults and adolescents. It has not been proven effective or safe for the routine prevention of HAE attacks. Safety and efficacy have not been established in children under the age of 13.

Pregnancy

There are no adequate and well-controlled studies on the use of Ruconest in pregnant women. In animal studies, no evidence of harm to the fetus was observed when the drug was administered at doses significantly higher than those used in humans. However, because animal studies do not always predict human response, Ruconest should be used during pregnancy only if clearly needed.

If you are pregnant or planning to become pregnant, discuss your treatment options with your healthcare provider to weigh the clinical necessity against any potential risks.

Breastfeeding

It is not known whether Ruconest is excreted in human breast milk. Because many drugs can pass into milk, caution is advised when Ruconest is administered to a person who is breastfeeding. When making a decision about treatment, healthcare providers and patients should consider:

• The developmental and health benefits of breastfeeding for the infant.
• The mother's clinical need for the medication.
• Any potential adverse effects on the breastfed child from the drug or from the mother's underlying condition.

Dosage Forms

Ruconest is provided as a lyophilized (freeze-dried) powder in a single-use glass vial. This powder must be reconstituted with sterile water before it can be administered.

Strengths

• Powder for Injection: Each vial contains 2100 International Units (IU) of C1 esterase inhibitor (recombinant).

The medication is administered as a slow intravenous injection over approximately five minutes.

FDA Approval and Indications

Ruconest (C1 esterase inhibitor [recombinant]) was approved by the U.S. Food and Drug Administration (FDA) in July 2014. It is indicated for the treatment of acute attacks in adult and adolescent patients with hereditary angioedema (HAE).

Limitations of Use

The FDA-approved labeling includes specific limitations for the use of Ruconest:

• It is not indicated for the chronic prevention (prophylaxis) of HAE attacks.
• The effectiveness of Ruconest has not been established in HAE patients experiencing laryngeal attacks.

Safety and Regulatory Considerations

Ruconest is produced using recombinant DNA technology in the milk of transgenic rabbits. Consequently, the FDA requires a contraindication for patients with a known or suspected allergy to rabbits or rabbit-derived products. Severe hypersensitivity reactions, including anaphylaxis, have been reported. There is currently no Risk Evaluation and Mitigation Strategy (REMS) program required for this medication.
