Boxed Warning: Heart Rhythm Risks
Thioridazine has been shown to prolong the QT interval, which is a measure of the heart's electrical cycle. This can lead to a life-threatening heart rhythm disorder and sudden death. Due to this risk, thioridazine is generally reserved for the treatment of schizophrenia in patients who have not responded to or cannot tolerate at least two other antipsychotic medications.
Dementia-Related Psychosis
Older adults with dementia-related psychosis treated with antipsychotic medications are at an increased risk of death. Thioridazine is not approved for the treatment of dementia-related psychosis.
Contraindications and Interactions
Thioridazine should not be used in combination with certain other medications that also affect the heart rhythm or interfere with the body's ability to process thioridazine. This includes:
- Specific antidepressants such as fluoxetine, paroxetine, and fluvoxamine.
- Certain heart rhythm medications (antiarrhythmics).

Additional Precautions
- Vision Changes: High doses of thioridazine have been associated with pigmentary retinopathy, which can cause blurred vision or impaired night vision.
- Low Blood Pressure: Patients may experience a sudden drop in blood pressure, especially when moving from a sitting or lying position to standing.
- Seizure Risk: This medication may lower the seizure threshold, increasing the risk of seizures in susceptible individuals.
- Blood Cell Counts: There is a risk of severe decreases in white blood cell counts, such as agranulocytosis or leukopenia.
- CNS Depression: Combining thioridazine with alcohol or opioids can lead to profound sedation, respiratory distress, coma, or death.
How to Take Thioridazine
