Chronic Hepatitis C Treatment

Vosevi is used to treat adults with chronic hepatitis C virus (HCV) infection. It contains three active ingredients—sofosbuvir, velpatasvir, and voxilaprevir—that work together to stop the virus from replicating in the body.

Targeted Genotypes

This medication is effective against all six major genotypes (strains) of the hepatitis C virus, including genotypes 1, 2, 3, 4, 5, and 6.

Previous Treatment Failure

Vosevi is often prescribed as a re-treatment option for individuals who have already completed a course of other direct-acting antiviral medications without being cured. This includes patients who previously used:

• Sofosbuvir or other specific antiviral drugs for certain genotypes.
• NS5A inhibitors, which are a specific class of hepatitis C medications.

Liver Condition

The medication is intended for patients who either do not have cirrhosis or have compensated (mild) cirrhosis. It is not recommended for individuals with moderate to severe liver impairment.

Mechanism of Action

Vosevi is a combination of three direct-acting antiviral (DAA) medications: sofosbuvir, velpatasvir, and voxilaprevir. These drugs work together to block the hepatitis C virus (HCV) from replicating, which eventually clears the infection from the body.

Each of the three components targets a different protein that the virus requires to complete its life cycle:

• Sofosbuvir is a polymerase inhibitor. It interferes with the enzyme the virus uses to copy its genetic material (RNA), effectively stopping the production of new viral particles.
• Velpatasvir is an NS5A inhibitor. It blocks a specific protein that the virus needs to assemble new virus particles and move them out of infected liver cells.
• Voxilaprevir is a protease inhibitor. It prevents an enzyme from cutting long viral protein chains into the smaller, functional pieces the virus needs to mature and reproduce.

By attacking the virus at three different stages of its development, Vosevi makes it very difficult for the virus to survive or develop resistance to the treatment. The goal of this therapy is to achieve a sustained virologic response, meaning the virus is no longer detectable in the blood several weeks after treatment is finished.

Common Side Effects

The most frequently reported side effects of Vosevi are generally mild and may include:

• Headache
• Tiredness or fatigue
• Nausea
• Diarrhea
• Difficulty sleeping
• Abdominal pain
• Decreased appetite

Serious Side Effects

Some patients may experience severe side effects that require medical intervention. Contact a healthcare provider immediately if you notice:

• Liver issues: Signs of worsening liver function include yellowing of the skin or eyes (jaundice), dark-colored urine, light-colored stools, or severe pain in the upper right side of the stomach.
• Hepatitis B reactivation: In patients who have had hepatitis B in the past, the virus may become active again during or after treatment.
• Slow heart rate: When taken with the heart medication amiodarone, Vosevi can cause a dangerously slow heartbeat (bradycardia). Symptoms include fainting, dizziness, or shortness of breath.
• Severe allergic reactions: Symptoms may include a widespread rash, swelling of the lips or tongue, and difficulty breathing.

Hepatitis B Virus Reactivation

Before starting treatment, healthcare providers screen for current or past hepatitis B virus (HBV) infection. In patients with both hepatitis B and C, the hepatitis B virus can become active again during or after treatment with Vosevi. This reactivation can lead to serious liver problems, including liver failure or death.

Liver Health Considerations

Vosevi is not recommended for patients with moderate to severe liver impairment (Child-Pugh B or C). Rare cases of worsening liver function or liver failure have been reported in patients with advanced liver disease. Monitoring for signs of liver injury, such as yellowing of the skin or eyes, dark urine, or abdominal swelling, is important during treatment.

Slow Heart Rate (Bradycardia)

Taking Vosevi with amiodarone, a medication used for heart rhythm, can cause a serious and potentially life-threatening slowing of the heart rate. This risk is higher for individuals also taking beta-blockers or those with existing heart conditions. Symptoms of a slow heart rate may include fainting, dizziness, chest pain, or shortness of breath.

Drug Interactions

Certain medications and herbal supplements can interfere with how Vosevi works or increase the risk of adverse effects. Vosevi should not be used with rifampin or St. John's wort. Other medications, such as certain seizure drugs or cholesterol-lowering statins, may also require monitoring or dosage adjustments.

Diabetes and Blood Sugar

Patients with diabetes may experience improved glucose metabolism during hepatitis C treatment, which can lead to low blood sugar (hypoglycemia). Close monitoring of blood sugar levels and potential adjustments to diabetes medications may be necessary.

Pregnancy

There is limited information regarding the safety of Vosevi use during pregnancy. Clinical trials in humans have not been conducted to determine if the medication poses a risk to the fetus or increases the chance of pregnancy loss. While animal studies using the individual components of Vosevi did not show evidence of harm at high doses, these results do not always predict human outcomes.

If you are pregnant or planning to become pregnant, it is important to discuss your treatment options with a healthcare professional. Vosevi is typically only recommended during pregnancy if the potential benefits are considered to outweigh the potential risks to the unborn baby.

Breastfeeding

It is not known if the components of Vosevi pass into human breast milk or if the drug has any effect on a nursing infant or milk production. Because the risks are not fully understood, individuals should consult with their healthcare provider to weigh the benefits of breastfeeding against the potential risks to the infant while taking this medication.
