Mechanism of Action
Zarxio is a man-made version of a natural protein produced in the body called granulocyte colony-stimulating factor (G-CSF). It belongs to a class of medications known as colony-stimulating factors.
Increasing White Blood Cells
The primary function of Zarxio is to act on the bone marrow to increase the production and release of neutrophils, a specific type of white blood cell. These cells are essential for the immune system to identify and destroy harmful bacteria and fungi.

Serious Complications

Kidney Injury: Glomerulonephritis (inflammation of the kidney filters) has been reported. Symptoms include puffiness in the face or ankles, blood in the urine, or decreased urination.
Capillary Leak Syndrome: This condition causes fluid to leak from blood vessels into body tissues. Symptoms include swelling, puffiness, decreased urination, and a feeling of fullness or bloating.
Aortitis: Inflammation of the aorta (the large blood vessel leaving the heart) may occur. Symptoms include fever, abdominal pain, and feeling tired.
Myelodysplastic Syndrome and Leukemia: In patients with certain conditions like chronic neutropenia, there is a risk of developing myelodysplastic syndrome (MDS) or acute myeloid leukemia (AML).
Monitoring and Contraindications
- Blood Cell Counts: Regular blood tests are performed to monitor white blood cell counts, as excessively high levels (leukocytosis) can occur.
- Contraindications: Zarxio should not be used by individuals with a history of serious allergic reactions to filgrastim or pegfilgrastim products.
Administration Methods
Zarxio is administered as an injection. It can be given in two ways:
- Subcutaneous injection: Injected into the fatty tissue just beneath the skin.
- Intravenous infusion: Delivered directly into a vein through a needle or catheter.
A healthcare professional may administer the medication, or you may be trained to perform subcutaneous injections at home.
How to Use

Pregnancy
There are no adequate and well-controlled studies regarding the use of Zarxio in pregnant women. Research in animals has shown that filgrastim products may cause adverse effects on the fetus when administered at very high doses, but these results do not always predict how the drug will affect human pregnancies. Because of the lack of human data, healthcare providers typically only prescribe Zarxio during pregnancy if the medical necessity justifies the potential risks.
Breastfeeding
It is not known whether Zarxio is excreted in human breast milk. Because many medications are passed through breast milk and could potentially affect a nursing infant, caution is recommended.
