Generalized Myasthenia Gravis (gMG)

Zilbrysq is used for the treatment of generalized myasthenia gravis in adults. It is specifically intended for patients who have anti-acetylcholine receptor (AChR) antibodies, which are identified through medical testing. This condition is a chronic autoimmune disorder that interferes with the way nerves communicate with muscles, leading to muscle weakness.

Target Symptoms

Mechanism of Action

In people with gMG, the immune system mistakenly attacks the neuromuscular junction, which is the area where nerve cells communicate with muscle cells.

The medication works by targeting the complement system, a group of proteins in the blood that are part of the body's natural immune defense. In gMG, this system becomes overactive and damages the surface of muscle cells, leading to weakness and fatigue.

Zilbrysq works through the following steps:

• Binding to C5: The drug binds specifically to a protein called complement component 5 (C5).
• Blocking Activation: By binding to C5, Zilbrysq prevents it from being broken down into smaller proteins (C5a and C5b) that would normally trigger further immune destruction.
• Preventing Muscle Damage: This action stops the formation of the membrane attack complex (MAC), which is the final step in the complement process that causes physical damage to the muscle membrane.

Common Side Effects

The most frequently reported side effects of Zilbrysq include:

• Injection site reactions, such as pain, redness, bruising, or swelling at the site of the shot
• Diarrhea
• Upper respiratory tract infections, such as the common cold or sinus infections
• Urinary tract infections

Serious Side Effects

Zilbrysq can cause serious side effects that require immediate medical care. These include:

• Meningococcal infections: This medication increases the risk of life-threatening meningitis. Seek emergency help for a sudden high fever, severe headache, stiff neck, nausea, vomiting, confusion, or sensitivity to light.
• Other infections: Because this drug affects the immune system, it may increase the risk of other serious bacterial, viral, or fungal infections.
• Allergic reactions: Symptoms may include a severe rash, hives, swelling of the face or throat, and difficulty breathing.
• Pancreatitis: Some patients may experience inflammation of the pancreas. Contact a doctor for severe pain in the upper stomach area that may spread to the back, often accompanied by nausea or vomiting.

Serious Meningococcal Infections

Zilbrysq increases the risk of serious and life-threatening meningococcal infections caused by Neisseria meningitidis. These infections can become rapidly fatal if not recognized and treated early. Because of this risk, Zilbrysq is only available through a restricted distribution program called the Zilbrysq REMS.

Vaccination Requirements

Individuals must be vaccinated against meningococcal infections at least two weeks before starting Zilbrysq. If treatment must begin urgently in an unvaccinated person, meningococcal vaccines should be administered as soon as possible, and the person must take protective antibiotics. Vaccination reduces but does not eliminate the risk of infection.

Monitoring for Infection

Healthcare providers and patients must monitor for early signs of meningococcal infection, which may include:

• Severe headache with nausea or vomiting
• Fever and stiff neck or back
• Confusion
• Muscle aches with flu-like symptoms
• Sensitivity to light
• A rash that does not fade when pressed

Medical help should be sought immediately if any of these symptoms occur.

Other Infections

Zilbrysq may increase susceptibility to other infections, including those caused by Streptococcus pneumoniae and Haemophilus influenzae type b (Hib). It is recommended that patients are up to date with vaccinations against these pathogens according to current guidelines.

Pancreatitis

Inflammation of the pancreas (pancreatitis) has been reported in patients using Zilbrysq. Treatment should be discontinued if pancreatitis is suspected. Signs include severe upper abdominal pain that may radiate to the back, nausea, and vomiting.

Contraindications

Zilbrysq must not be started in individuals with an ongoing, unresolved serious Neisseria meningitidis infection.

Pregnancy

It is not currently known if Zilbrysq will harm an unborn baby. There have not been enough studies in pregnant humans to determine if the medication is safe to use during pregnancy. Breastfeeding

Zilbrysq is available as a solution for subcutaneous injection. It is provided in single-dose prefilled syringes.

This medication is intended for self-injection after a healthcare professional has provided proper training on how to prepare and inject the dose. Each prefilled syringe is used once and then discarded.

FDA Approval and Indications

Boxed Warning

To mitigate this risk, patients must be vaccinated against meningococcal disease at least two weeks before starting the medication. If treatment must start sooner, healthcare providers must prescribe preventive antibiotics.

REMS Program

Because of the risk of serious infection, Zilbrysq is only available through a restricted program called the Zilbrysq Risk Evaluation and Mitigation Strategy (REMS). This program requires:

• Patients to be enrolled in the program and carry a patient safety card at all times.
