Approved Uses

Zykadia is indicated for the treatment of patients with metastatic non-small cell lung cancer (NSCLC). It is specifically used for tumors that are determined to be anaplastic lymphoma kinase (ALK)-positive by an FDA-approved test.

• First-line treatment: For patients who have not previously received treatment for their metastatic lung cancer.
• Subsequent treatment: For patients whose cancer has progressed or who could not tolerate other treatments, such as crizotinib.

Serious Side Effects

• Lung problems: New or worsening cough, shortness of breath, chest pain, or fever.
• Heart rhythm issues: Very slow, fast, or irregular heartbeat, or feeling like you might pass out.
• High blood sugar: Increased thirst, frequent urination, or blurred vision.
• Pancreatitis: Severe pain in the upper stomach that may spread to the back, often accompanied by nausea or vomiting.

Gastrointestinal Toxicity

Severe diarrhea, nausea, vomiting, and abdominal pain are common during treatment. Healthcare providers may recommend supportive care or adjust the dosage if these symptoms become severe.

Liver Toxicity

Ceritinib can cause elevations in liver enzymes, which may indicate liver damage. Regular blood tests are typically used to monitor liver function before and during treatment.

Lung Problems

Severe or life-threatening inflammation of the lungs, such as interstitial lung disease or pneumonitis, has been reported. Symptoms such as new or worsening shortness of breath, cough, or fever require immediate evaluation.

Heart Rhythm and Rate

This medication may cause a slow heart rate (bradycardia) or changes in the heart's electrical activity, known as QT prolongation. Monitoring with electrocardiograms (ECGs) and electrolyte checks is often necessary, especially for those with pre-existing heart conditions.

Hyperglycemia

High blood sugar levels can occur, even in individuals without a history of diabetes. Blood glucose monitoring is typically recommended before starting and during treatment. If hyperglycemia develops, healthcare providers may initiate or adjust glucose-lowering medications.

Pancreatitis

Inflammation of the pancreas has occurred in patients taking this medication. Monitoring of lipase and amylase levels in the blood is used to detect this condition.

Embryo-Fetal Toxicity

Based on its mechanism of action, this drug can cause harm to a fetus. Effective contraception is recommended for patients of reproductive potential during treatment and for a specified period after the final dose.

Pregnancy

Zykadia can cause harm to an unborn baby. Patients should inform their healthcare provider if they are pregnant or planning to become pregnant. A pregnancy test is usually performed before starting treatment to ensure the patient is not pregnant.

Because of the potential risks, effective contraception is necessary for both men and women during and after treatment:

• Females: Use effective birth control during treatment and for at least 6 months after the final dose.
• Males: Use effective birth control (such as condoms) during treatment and for at least 3 months after the final dose if your partner is of reproductive potential.

Breastfeeding

It is not known whether Zykadia passes into human breast milk. Due to the potential for serious adverse reactions in a nursing infant, breastfeeding is not recommended during treatment and for at least 2 weeks after the last dose of the medication.
