Dupilumab

Approved Indications

Atopic Dermatitis (Eczema)

Chronic Rhinosinusitis with Nasal Polyposis

This medication is used in combination with other treatments for adults whose chronic rhinosinusitis with nasal polyps is not adequately controlled.

Eosinophilic Esophagitis

It is used to treat adults and pediatric patients weighing at least 15 kg with eosinophilic esophagitis, a condition where allergic inflammation damages the esophagus.

Prurigo Nodularis

Dupilumab is approved for adults with prurigo nodularis, a chronic skin condition characterized by intense itching and firm skin lumps.

Chronic Spontaneous Urticaria

It is used to treat chronic spontaneous urticaria in adults and adolescents whose symptoms continue despite the use of H1 antihistamine treatment.

Mechanism of Action

Dupilumab belongs to a class of medications known as monoclonal antibodies. It is designed to target and block the action of two specific proteins in the immune system: interleukin-4 (IL-4) and interleukin-13 (IL-13).

These proteins are signaling molecules that play a major role in type 2 inflammation. In people with certain conditions, the body produces too much of these proteins, leading to an overactive immune response. This inflammation can cause various symptoms depending on where it occurs in the body, such as:

• Skin irritation and itching in atopic dermatitis.
• Airway swelling and mucus production in asthma.

By binding to a specific part of the cell surface called the IL-4 receptor alpha subunit, dupilumab prevents both IL-4 and IL-13 from sending signals to the cells. This helps to quiet the inflammatory process and reduce the symptoms of the underlying condition.

Common Side Effects

The most frequent side effects reported with dupilumab include:

• Redness, pain, swelling, or itching at the injection site
• Eye problems, such as redness, itching, or dryness
• Inflammation of the eyelid or the surface of the eye
• Cold sores on the lips or in the mouth
• Joint pain
• Sore throat
• Trouble sleeping

Serious Side Effects

• Signs of a severe allergic reaction, such as hives, rash, swelling of the face or throat, or difficulty breathing
• New or worsening eye symptoms, including eye pain or changes in vision
• Symptoms of blood vessel inflammation, such as fever, chest pain, shortness of breath, or a tingling sensation in the arms or legs

Allergic Reactions

Asthma and Steroid Use

Dupilumab is not a rescue medication and should not be used to treat sudden asthma attacks or acute breathing distress. If you are taking oral or inhaled corticosteroids, these should not be stopped or reduced suddenly when starting dupilumab. Any changes to steroid dosages must be managed gradually by a healthcare professional to avoid withdrawal symptoms or the worsening of other conditions.

Eosinophilic Conditions

In some cases, people with asthma being treated with this medication have developed inflammation of the blood vessels (vasculitis) or a high level of certain white blood cells (eosinophilia). This often happens when oral steroid doses are being reduced.

Parasitic Infections

Because this medication affects the immune system, it may weaken the body's ability to fight certain parasite (helminth) infections. Pre-existing infections should be treated before starting the medication. Vaccinations

Joint Pain

Some people have reported new or worsening joint pain while using this medication. In some instances, this pain has led to the discontinuation of the drug.

Pregnancy

There is limited data regarding the use of dupilumab in pregnant individuals. It is not yet known if the medication can cause harm to an unborn baby or lead to pregnancy complications. Because dupilumab is a monoclonal antibody, it is expected to cross the placental barrier, meaning it could potentially reach the fetus.

• Asthma: For patients 6 years of age and older with specific types of moderate-to-severe asthma.
• Chronic Rhinosinusitis with Nasal Polyposis: For adults with inadequately controlled disease.
