Primary Biliary Cholangitis involves an immune-mediated process where the body's defense system mistakenly attacks the small intrahepatic bile ducts. This damage can lead to a buildup of bile, a fluid essential for digestion and waste removal, which may result in inflammation and long-term liver injury.

Formerly known as primary biliary cirrhosis, the condition primarily affects middle-aged women and is considered a slow-progressing disease. Over time, the persistent cholestasis and resulting tissue damage can lead to the development of permanent scarring within the liver.
